Pregnancy card

The pregnancy certificate is one of the basic documents of a pregnant woman . A woman should always have it with her in case of sudden health complications. From the card, the staff of the given medical facility will learn the most important information about the health of the pregnant woman and the state of the pregnancy. There are many types of cards, which differ in graphic design or different differentiation of boxes. However, the volume of basic information remains the same.

The first page of the ID card[edit | edit source]

On the first page we find the personal data of the pregnant woman . Next, the date of the first day of the last menstrual period (PM) and the expected date of delivery (TP). Some doctors also record the date of delivery from an ultrasound examination here, which is then more accurate. In the lower half of the first page, we also find information about previous pregnancies and their possible complications. Appointments for doctor visits are also written here.

Record of pre-illness and current pregnancy[edit | edit source]

The record covers the following four pages. Here we will find important data from the pre-disease and the course of the current pregnancy , records of special samplings and examinations or also prescribed medicines , and so on. While the third and fourth pages of the card are very important, they contain a table where the doctor records the current subjective state of the pregnant woman as well as the results of the objective examination.

Types of examinations recorded in the table:

  • pressure test
  • the presence of swelling,
  • found in the birth canal,
  • fetal position.

In the upper part of the table, the results of standard samplings during pregnancy (e.g. blood group), as well as the dimensions of the pelvis or the woman's height before pregnancy , are entered .

The fifth and sixth page of the ID card[edit | edit source]

The following double page continues the graphs and tables on the previous pages and complements them. However, filling in the tables on the fifth and sixth pages is not necessary.

Records of other examinations[edit | edit source]

On the following pages, we will find records of ultrasound examinations from the third, fifth and eighth months of pregnancy , as well as the results of cardiotocographic examinations (CTG).

The last double page is used to record the results of the blood count, data on hospitalizations during pregnancy or examinations by other specialist doctors.
