Paraphimosis

From WikiLectures

Paraphimosis

Paraphimosis jis an acute painful condition in urology, in which the foreskin is pulled into the sulcus coronarius the glans penis (most often due to an iatrogenic cause, e.g. during catheterization of the bladder or examination of the penis), which leads to strangulation of the glans. Venous and lymphatic drainage deteriorates, swelling of the glans penis gradually develops. With increasing edema, arterial blood flow is also impaired. Due to the increasing swelling, the foreskin cannot be pulled back. This condition can lead to ischemia.

Unlike this conditionphimosis is a non-urgent situation in which the foreskin cannot be pulled behind the glans penis.

Therapy[edit | edit source]

The solution is manual reduction of paraphimosis in the first hours. Pressure on the foreskin reduces the swelling of the glans penis, after which it is possible to try to reposition the foreskin manually. If this fails, an incision is necessary. Since this is a very painful condition, it is necessary to give the patient a sufficient dose of analgesics.
