Genetic questionnaire

Genetic questionnaire (also pedigree questionnaire) is a special form that is used in clinical genetics when compiling a family tree. In ambulatory genetic practice, the use of a questionnaire (which the proband and his family fill out in the comfort of their home before the actual consultation) is advantageous, as it speeds up the compilation of the family tree in the ambulatory. The forms of the questionnaire are different (and each genetic workplace usually has its own form), but the basis is to find out information about the proband, his family members and their health status in a clear way.

The questionnaire serves the geneticist as an aid for compiling a family tree. It can never completely replace a detailed interview with the proband.
